Archives & Questionnaire Requests*If you have a custom form you need completed by Associa, please follow the below registration/log-in instructions and then select the HOA Questionnaire (aka Lender’s Disclosure/Condo Questionnaire) option and add-on the custom form to the order. Once you have placed your order, you must email the form to associahawaiicommunityarchives@associa.us (including the order number) as the order will not begin processing until you have submitted your custom form through for order.

If you have previously ordered documents from Associa Hawaii, log into the green box using your registered email address and password.
First time user:
- Click on “First time user? Please click here to register”
Note: Community Archives services over 60 different management companies – If you have not registered for Associa Hawaii (specifically), you will need to do so. - You will then be prompted to fill out a registration form.
- Once you complete the registration, you will then be prompted to type the first three letters of the community name in the box on the screen. It will then populate and your community should be provided for you to select at this time. (If the community does not populate for you, please contact Associa Hawaii to confirm the community name is correct/be sure you see Associa Hawaii listed at the top of the page.) Please note: If you press enter or space key when attempting to pull up a community name, you will receive an error message. Type the first three letters of the community name in the box and wait for the information to populate.
- Once you have selected the desired association, a list of products will be displayed for you to select from.
*If you need the documents sooner than 10 business days, please select the expedite deliver option prior to checkout – expedited orders should be received within 24 business hours (3 business days)
*Orders will not begin processing until the payment method has been applied to the order. We recommend entering the credit card prior to checkout to avoid delays – If you select 3rd party payment a link will need to be emailed to you to add the payment method to the order. If you are paying with a check, please note it can take several days for the check to be received and applied to the order.
*when filling-in the order information, if there are any mandatory fields you do not have information for (ie buyer’s information), you would enter “UNKNOWN” or “NA”.
